DSIS Registration Material

Complete and return the forms mailed to you, to the DSIS office by August 18, 2011:

• Master Agreement
• DSIS Student Emergency Card
• DSIS Libraray Patron Form

DSIS Student Assignments

There are four assignments to complete prior to meeting with your teacher:

• a quilt square (use piece of construction paper mailed to you)
• a poem
• Student Learning Outcome (SLO) assignment (gr. 7-12 only)
• essay (gr. 7-12 only)

• Returning students bring all completed assignments to your first meeting with your teacher.
• New students (gr. 7-12) must come to DSIS on August 22nd at 11:00 a.m.or 1:00 p.m. to complete their essay on site. Bring the other three assignments to your first meeting with your teacher.

Complete and sign the Summary Signature Form from the DJUSD website

**NEW SECONDARY STUDENTS There is a mandatory orientation for students and their parents/guardians on Monday, August 22nd at 6:00 p.m. in the East Conference Room at the Davis Joint Unified School District Office. This will be an opportunity to learn more about DSIS and ask any questions you might have.
