KINDERGARTEN REGISTRATION:

TRANSITIONAL KINDERGARTEN (TK) Fact Sheet:
In order to register your
incoming kindergartener, we need the following documents:
- Completed registration packet.
- Proof of residence, such as a PG&E bill, rental/lease agreement, escrow papers, homeowners insurance.
- Immunization records.
- Birth certificate or passport. (Your child must be 5 yrs. old by
11/1/12)
- Parent Picture ID (drivers license, passport)
- ***Also note that a physical examination form has to be completed by your doctor within six months of your child entering kindergarten.
Information we require to
enroll your 1st-6th grader:
- Completed
DJUSD Registration Form
- Verification
of Residence (lease, rental agreement, escrow papers or PG&E
bill)
- Proof
of immunizations
- Parent
Picture ID (drivers license, passport)
- Student
Birth Certificate
- Name,
address, phone and fax number of most current previous school
