Business Services Forms

This section of the Business Services web page is designed to provide the reader with copies of the many forms used by district staff at the schools and in the District Office.   • School-Connection Organization Authorization Form
  • Tax Exemption Forms Please refer to the Tax Exemption Guidelines for an explanation of which Measure H application to submit.  You will only submit the one for SSI or SSDI, if you are receiving those benefits, and you must submit an application each year. Otherwise, use the Senior Citizen Tax Exemption Application for Measure H. If you are already receiving a Senior Citizen Exemption for Measure H, you do not need to reapply, providing the home is still your primary residence and you are still the owner. 

If you are already receiving an exemption for Community Facilities District No. 1 (CFD No. 1), you do not need to reapply, providing the home is still your primary residence and you are still the owner.
