  Step 2)  After you read the items listed in Step 1, you must print, then sign this form and return it. Summary Signature Form (required)

  Step 3)  You may want to print, complete and turn in:

• Free and Reduced Cost Lunch Application (recommended)

• Restriction from Displaying Student Work    (optional)

• Restriction from Release of Student Information (optional)

 Step 4)  You may want to visit the district website (informational) to get more information about Low-cost Preschool and Childcare, the GATE Program, Student Accident Insurance, Medi-Cal Insurance, Instrumental Music Program, Information about Pesticide Use.

SITE FORMS

• Emergency cards (required) will be distributed on August 21st.  Please bring your other contact information (phone numbers for doctor, dentist and emergency contacts, insurance info) so you will be prepared to complete and sign.
