Important Enrollment Information

Benefits Enrollment for New Employees

To activate your benefits, you must submit your completed enrollment forms to Fiscal Services.

When coverage begins:
Medical, dental, and vision coverage will start on the 1st of the month following your start date, as long as:

  1. Fiscal Services receives your forms by the 10th of the month prior, and
  2. A paycheck is on record.

Why a paycheck matters:

  • Benefits cannot begin until a paycheck has been created, because both you and the District contribute to the health premium through payroll deductions.
  • This requirement applies to all employees, both Classified and Certificated.

Example for certificated employees:

  • An employee starting August 21 will not receive their first paycheck until September 30.
  • Because of this, the earliest benefits can begin is October 1.

Examples of enrollment timing:

  • If your paperwork is received by September 10, and you have a paycheck on record, your coverage will begin October 1.
  • If your forms are received after September 10, your coverage will begin November 1.

CalPERS Health Plans and Resources
Important information to know:
  • If you turn in an "Enrollment" form by the 10th of the month, your changes will start on the 1st of the next month. If you turn it in after the 10th, your changes will start the month after that.
  • Filling out a "Waiver of Health Coverage" form does not cancel your current health benefits. If you want to stop your health coverage, you must complete a new Informed K-12 "Enrollment" form.
  • Do not make changes directly on the CalPERS website. The district does not receive updates from CalPERS, so your payroll deductions will not be adjusted.
  • To ensure your changes are processed correctly, always submit a new Informed K-12 "Enrollment" form through the district.
  • Click "Enrollment" at the top of the webpage to find enrollment forms.

Important Documents Needed to Activate Coverage

Enrollment forms are required to activate coverage or to change existing coverage.

Once forms are submitted and processed, the employee will receive a read-only copy of any form(s) submitted as confirmation of enrollment.

Employees that are eligible for benefits but choose to waive health coverage must complete the "Waiver of Health Coverage" form found at top of page under "Enrollment".

Required documents to add spouse or dependents:

  • Dependents: Copy of the birth certificate for all dependents under 26; medical certification for all disabled dependents.
  • Spouse or domestic partner: Copy of marriage license, domestic partnership certification,
    or court order.
Dental - Delta Dental

DJUSD employees are offered dental benefits through Delta Dental PPO of California. Delta Dental does not mail welcome kits or ID cards.

Employer Medical Contributions

Davis Joint Unified School District, like many districts in California, has a cap on the amount we contribute to each employee group. Below is what we offer by group. Monthly amounts below reflect the benefit for 1 full-time equivalent (FTE) employment and are prorated for total FTE over .50 to 1.0.

Health monthly Benefit Caps amount are calculated based off of a 12 month pay cycle.
